5. Increase understanding of lived experience of perinatal SUD and the peer/doula role in supporting patients and care teams
Diana Louise Smith, CNM, MPH is a certified nurse-midwife based in Portland, Oregon and a subject matter expert on perinatal substance use. In her work with Project Nurture, Diana provides integrated substance use treatment and maternity care in a model that centers lived experience and integrated physical and behavioral healthcare during pregnancy and through the year postpartum.

Coordinated Prevention of Congenital Syphilis: Open Webinar Series

The Oregon Perinatal Collaborative (OPC), in partnership with Comagine Health and with funding from the Oregon Health Authority, is leading a statewide initiative to prevent congenital syphilis through coordinated screening, treatment, and follow-up of pregnant patients in emergency department and community settings. This on-demand series presents setting-specific best-practice guidance developed with emergency departments, local public health authorities, and community partners across Oregon.

OPC Coordinated Prevention of Congenital Syphilis: Emergency Departments

A majority of congenital syphilis cases in Oregon occur in pregnant patients who have had no prenatal care or entered care within 30 days of delivery, yet many were seen in an emergency department at least once during pregnancy. This makes the ED a critical, and frequently missed, opportunity for prevention. Drs. Irina Cassimatis (Maternal-Fetal Medicine) and Jack Marshall (Emergency Medicine) will review:

  • systematic identification of pregnancy in ED triage units
  • syphilis screening using traditional and reverse-sequence algorithms
  • capture of patient contact information at the point of ordering to enable public health follow-up

OPC Coordinated Prevention of Congenital Syphilis: Local Public Health Departments

Local public health authorities are central to ensuring that a reactive syphilis result in pregnancy leads to timely staging, adequate treatment, and partner management. This session examines strategies to strengthen LPHA collaboration with emergency departments and community organizations, including:

  • designated, monitored referral contacts
  • streamlined staging and treatment orders for public health nursing
  • field-based testing and treatment
  • approaches to benzathine penicillin G access and expedited partner treatment

OPC Coordinated Prevention of Congenital Syphilis: Community Organizations

Community-based organizations are often the most consistent point of contact for pregnant people facing housing instability, substance use, or other barriers to prenatal care. This session describes how CBOs can contribute to congenital syphilis prevention through risk identification, facilitated linkage to testing and treatment, and support for completion of multi-dose treatment regimens. It also covers structured partnerships with local public health for locating and re-engaging patients. The session highlights outreach models with demonstrated success in Oregon, including integration with syringe services programs, shelter- and encampment-based outreach, and incentive-based engagement strategies.

OPC Coordinated Prevention of Congenital Syphilis: Prenatal Providers

Prenatal care is the first and best opportunity to prevent congenital syphilis, but missed screening, delayed or incomplete treatment, and confusing test results can lead to preventable cases. This session reviews current state recommendations for syphilis in pregnancy and covers:

  • Frequency of syphilis screening in pregnancy
  • Interpreting treponemal and nontreponemal serologies, including discordant results under reverse-sequence algorithms
  • Staging, treatment timing, and adequacy relative to delivery
  • Managing missed doses, partner treatment, and serologic follow-up

It also addresses how prenatal care teams can coordinate with local public health to support continuity for patients first identified in the ED or on labor and delivery, particularly those with limited or late prenatal care.

On Nov. 4, 2025, the Oregon Perinatal Collaborative coordinated a webinar on informed choice and respectful care with Dr. Nicholas Rubashkin of the University of California San Francisco (UCSF) and Human Rights in Childbirth. We have had many requests from OPC partners, especially those of you working on community birth transfer improvement, for more focused continuing education and this area, and we are excited to share this case study-based webinar highlighting best practices for common areas of conflict or concern in prenatal and labor care.

Oregon Doula Workforce Analysis

On December 1st, 2025, the Oregon Perinatal Collaborative coordinated a webinar that introduced key findings and recommendations from the recently published Oregon Doula Workforce Analysis. Representatives from CORE, who completed the study and the Oregon Doula Association, who were part of the planning and supported the connections into the community were on the call as well to share about related work and participate in a group discussion about the recommendations.

Severe Hypertension Initiative Webinars

From June 2024-June 2025, hospital teams across Oregon participated in a quality initiative to implement the Severe Hypertension in Pregnancy and Postpartum toolkit. OPC Severe Hypertension Initiative - Final Webinar

Hypertension for Doulas Webinar - On Demand
